What it means
Flame detection works by rectification, passing a small current through the flame to ground, so the control verifies its electrical environment before operating. This code means that check failed. York's list is broader than most manufacturers': as well as grounding and reversed line polarity, it names reversed transformer leads specifically. That third item matters because it is a wiring error made during installation or a transformer replacement, and it would not occur to most people to check it. All three are wiring faults rather than component failures, and all three almost always follow electrical work.
